The Maryland Immunization Regulation requires that the immunization record list all immunizations with the dates of each vaccination, it must be signed by a physician or health department official, and it must be approved by the admitting school. Immunizations must be completed before students attend school.
Children who live in Maryland and are 5 years old on or before September 1, 2017 are required to attend public school, unless they are educated in a private school or other alternative educational setting recognized by Maryland education bylaws. Parents selecting an alternative educational program for kindergarten that is not recognized by the Maryland State Department of Education, such as a full-time licensed daycare center, a full-time registered family daycare home, or a Head Start program for 5 year olds, must file their intent to enroll their 5-year-old child in an alternative program setting with the local school system before the opening of school.
Parents who believe that their child is not developmentally ready for kindergarten may request a waiver to delay kindergarten for one year by writing a letter to the Superintendent of Schools before the opening of school in the year in which the child becomes 5 years old. A waiver request should include the child’s full name and date of birth and must be filed prior to the beginning of the school year.
All students entering kindergarten must be 5 years old on or before September 1, 2017.

2. Immunization Record
3. Two (2) Proofs of Residence

also have two proofs of -Bank Statement or
